Information Saves Lives!
As more and more people are diagnosed with medical
conditions, as more and more people are taking medications
on a regular basis, it is more important than ever for us to be
able to effectively communicate our medical information to
medical professionals.  This is especially true during a
medical emergency.
According to the American Assn. of Family Physicians
(AAFP), “1 in 2 Americans have at least 1 chronic medical
condition and 60 million people in the United States suffer
from multiple chronic medical conditions.”   An article from
the AP clearly notes that "more than half of all insured
Americans are taking prescription medicines regularly for
chronic health problems.”
The AAFP adds;
“The Single Most Important Way You Can Help To Prevent
Errors Is To Be An Active Member Of Your Health Care
Team…
Keep Your Health Care Team Informed …
Do Not Assume That Everyone Knows Everything They Need
To.”
Agencies, Organizations, Doctors, Pharmacists, Hospitals…It
seems as though everyone is trying to get the message to us:
o        "The success of improving health care depends
directly on patients' involvement
in their care.” -The Institute of Medicine's "Crossing the
Quality Chasm"
o        "Keeping your own personal health record can allow
you to receive faster, safer
treatment and care in an emergency.” -American Health
Information Management Association
Please, take a moment to think about what you would do if
you or a loved one had a medical emergency right now.  If
you had to call 911 today, if paramedics came into your
home to save your life or your loved one's life, would you be
able to "make sure all medical professionals have the
information they need to give you the best care possible?”
For information on how you can make sure all your
information is ready for when you need it most, please look
